They provide documented evidence that your client has given you authorisation to carry out a Tatti Lash treatment and confirms that they understand the nature and method of the treatment.

Client Record Cards confirm that your client’s medical background and suitability has been assessed and that they have been informed of and agree to follow aftercare instructions and maintenance. 

Our Client Record cards have a section to help you thoroughly assess the lashes making it easier than ever to choose the correct curl and thickness for your client and allowing time to discuss the desired look while doing so.

Client Record Cards are required for all Tatti Lashes & Oh So Pro treatments for first time clients and are to be reviewed/updated at every appointment. We recommend a new card to be filled out if it has been 6 months+ since you last seen your client or if the medical history changes and another patch test has been performed. 

Please note : This is a downloadable PDF that you can keep forever. You can use this digitally or print them. You will be sent the PDF to download in your order confirmation.

Order before 9pm to receive your order the following day (Monday - Thursday, UK only). Any orders placed after 7pm on Thursday will be delivered on Monday. Any orders placed after 7pm on Saturday will be processed on Monday for Tuesday delivery (as it is a next working day service).
